This form submits to the IT Accessibility Office (OITA) to report disability barriers. OITA cannot fix bug issues within the LMS, answer course questions, or give access to courses or course content.

If your issue is in regards to not having access to particular portions of your online course, or broken links, please contact the Help Desk through the following options:
